Team Prevent

Team Prevent is a Centers for Disease Control and Prevention (CDC) evidence-based healthy lifestyle change program. The goal of the program is to help participants eat healthier, get at least 150 minutes of physical activity per week and lose at least 5 to 7% of their body weight.  In the process of learning these healthier lifestyle habits, participants not only lose weight but also decrease the risk of developing type 2 diabetes, heart disease, and stroke. The program can be completed in either an in-person format or an online format.

 

Many worksites are seeing the benefits of this program and are now providing the program to their employees at no cost or at a reduced cost.  Check with your employer to see if the program is covered or to inquire about their interest in offering the program to employees.  For employers interested in offering CoachShellie’s Team Prevent Program, click on Corporate Wellness for more information.

 

Description of In-person Program

Next program starts Wednesday, April 26, 2017, 5:00PM to 6:00PM at Early Learning Village (Frankfort, KY)

  • CDC evidence-based program
  • Participants reduce body weight by 5 to 7% within first 4 months
  • Participants increase physical activity to 150 minutes per week
  • Led by a trained wellness and lifestyle coach
  • 16 weekly in-person sessions
  • Wellness/lifestyle coach records each participant’s weight, privately, at the beginning of each session
  • Each person provides coach access to their food tracker
  • Coach provides feedback on food tracker
  • As a group, discuss successes and challenges from the last week
  • Each session has a lesson which the coach and participants go over in class
  • Each lesson includes education about a certain topic such as physical activity, tips on eating out, how to handle slip-ups, etc.
  • As a group, discuss personal experiences with the topic at hand and work on a plan to handle challenges that may come up
  • At the end of 16 weeks, there will be 8 monthly sessions that go into more depth of certain topics and will focus more on sustaining and maintaining a healthy lifestyle.

 

Description of Online Program

Ongoing Enrollment

  • CDC evidence-based program
  • Participants reduce body weight by 5 to 7% within first four months
  • Participants increase physical activity to 150 minutes per week
  • Led by a trained wellness and lifestyle coach
  • 16 weekly lessons are sent through an online app to participants
  • Participants will use an app to privately record their daily weight, food and physical activity
  • Coach will provide private comments and tips related to participant journal entries
  • At the end of 16 weeks, there will be 8 monthly lessons that go into more depth of certain topics and will focus more on sustaining and maintaining a healthy lifestyle
  • Optional online group discussion for each session

 

Program Qualifications

  • At least 18 years old
  • Overweight (BMI ≥25); or
  • Have been diagnosed with pre-diabetes within the past year; or
  • Have been previously diagnosed with gestational diabetes.

 

Kentucky Employee Health Plan Requirements (KEHP/Anthem – for state employees and public school staff)

  • Complete at least 4 lessons
  • After 16 classes, HumanaVitality members receive 350 vitality points
